1-Divyang Pension:- 

To get the benefit of the scheme following documents are required:-
 (a)Income certificate (from 46080/-annual in urban area and 56460/-annual in rural area
 (b) Certificate of Disability (whatever the percentage of disability)
 (c) Caste certificate (if coming under SC/ST)
 (d) Photocopy of Aadhar card
 (e) Photocopy of the passbook of the nationalized bank
 (f) Passport size photograph.

2-Leprosy Pension Scheme:- To get the benefit of the scheme following documents are required:-
(a) Income certificate (from 46080/-annual in urban area and 56460/-annual in rural area)
(b) Certificate of Disability (of 40% or more granted by the Chief Medical Officer)
    Permanent Disability Certificate)
(c) Caste certificate (if coming under SC/ST)
(d) Photocopy of Aadhar card
(f) Photocopy of the passbook of the nationalized bank
(g) Passport size photograph.

3-Marriage Incentive / Award Scheme:-To get the benefit of the scheme following documents are required:-
(a) Marriage Registration Certificate from the Registration Office.
(b) Income certificate (issued from husband and wife tehsil)
(c) Joint Account (Husband and Wife)
(d) Aadhar Card (Husband and Wife)
(f) Caste certificate (if coming under SC/ST)
(g) Marriage Card (attested by Gram Pradhan/councilor)
(h) Joint photograph of husband and wife.

4- Divyangjan shop construction / operation scheme:-To get the benefit of the scheme following documents are required:-
(a) Income certificate (not more than twice the poverty line)
(b) Certificate of Disability ( 40% or more /Permanent Disability Certificate granted by the Chief Medical Officer )
(c) Caste certificate (if coming under SC/ST)
(d) Photocopy of Aadhar card
(f) Photocopy of the passbook of the nationalized bank
(g) Passport size photograph.
(h) Photocopy of Aadhar card of two guarantors.

5- Artificial Limbs / Assistive Devices Scheme: – To get the benefit of the scheme following documents are required:-
(a) Income certificate (from 46080/-annual in urban area and 56460/-annual in rural area)
(b) Certificate of Disability ( 40% or more /Permanent Disability Certificate granted by the Chief Medical Officer )
(c) Caste certificate (if coming under SC/ST)
(d) Photocopy of Aadhar card
(h) Passport size photograph.

Equipments to be provided:- Tricycle , Wheelchair, Hearing aids , blind stick , walking stick ,Smartcane ,crutches , Leprosy Kit.

5-Grant for Medical Surgery:- Application form submitted to the applicant for surgery in the office of the undersigned After that, the applicant will have to go to the concerned hospital for surgery after the examination. Surgical procedure will be done.

6- State / National Award Scheme:- Persons with disabilities or such government and non-government persons who are doing highly specialized work in the field of disability can submit undersigned application in the prescribed format in the office. A cash prize 25000 / and citation is provided in the said scheme.
